In the Witcher 3 next-gen update, you have to go through a few steps to move your save game from the PS4 to the PS5. The first step is to update the PS4 version to version 4.0.0.

Then, go back into the game and save. Please don’t start a new game. Now, open the game on the new generation and turn on cross-progression there as well.
You should now be able to load the save file you just made. On PS5, you may have to start a new game to get the option to use cross-progression. So, that’s the end of it. Not really, no.
